No matter where you are in your caregiving journey — starting to plan; taking care of a family member in your home, in a facility, or from a distance; or managing end-of-life caregiving responsibilities — having resources at your fingertips will make the process easier.

AARP developed this family caregiver guide with you, the caregiver, in mind and as a starting point to help you find the services and support you might need throughout your journey.

Caregiver Support

Disabilities Aging and Independent Living Caregiver Programs (802-241-2401)
Supports family caregivers through grants that may be used to pay for in-home care, respite care, homemaker services and adult day services.

Finances

Senior Community Service Employment Program (800-439-3307 | 802-524-3200)
Community service and work-based job training program for older adults provides training for income-eligible, unemployed job seekers age 55-plus. Participants gain skills and knowledge and receive modest pay for working for nonprofit organizations.

Supplemental Nutrition Assistance Program (800-479-6151)
Eligible households receive food assistance through an Electronic Benefits Transfer card to cover a portion of their food budget, as well as nutritional education.

Vermont Department of Labor (802-828-4000)
Four major divisions — Workforce Development, Labor Market Information, Unemployment Insurance and Workers’ Compensation and Workplace Safety — provide job-searching resources and training programs.

Legal

Vermont Legal Aid (800-889-2047)
Nonprofit law firm dedicated to providing civil legal services for adults 60-plus, low-income residents or people with disabilities.

Health

Alzheimer’s Association (802-316-3839; 800-272-3900 24/7)
Provides people with Alzheimer’s disease or other forms of dementia and their caregivers a 24-hour telephone helpline, family education and outreach programs, financial and legal planning information, support groups and care navigation tools.

Dartmouth-Hitchcock Neurology Memory Clinic (603-650-5104)
Diagnoses age-related memory and thinking disorders and provides treatment, support, education programs and more to patients and their caregivers.

Memory Program, University of Vermont Medical Center (802-847-1111; 888-362-3242)
Provides evaluation, diagnosis and treatment of age-related memory disorders, such as Alzheimer’s disease, and management of the conditions that may produce these disorders.
 
Support and Services at Home (802-863-2224 | 800-253-0191 TTY)
Coordinates social service agencies, community health providers and nonprofit housing organizations to support Medicare-eligible older adults and individuals with special needs who choose to live independently at home.

VNAs of Vermont (855-484-3862; 802-229-0579)
Regardless of ability to pay, provides home health and hospice services by nurses, speech therapists, physical and occupational therapists, wound care specialists, social workers, home health aides, personal care attendants and others.

Housing and Care Facilities

Choices for Care (800-642-5119)
Long-term care services program provides care and support to eligible residents at least 18 years old who require a nursing home level of care or assistance with daily living activities.

Long-Term Care Ombudsman Program (800-889-2047)
Provides consumer protection advocacy services to residents of long-term care facilities such as nursing homes, assisted living facilities, specialty care facilities and boarding homes. Anyone can use an ombudsman to report a concern and get help resolving problems, including residents, residents’ family members and friends, and long-term care facility employees.

State Housing Authority (802-828-3295 | 800-798-3118 TDD; 800-820-5119 message line)
Provides affordable housing initiatives for older adults, young families, those who are homeless and those with special needs.

Addison County Home Health and Hospice (802-388-7259; 800-639-1521)
Provides long-term nursing care, adult home care, habilitation, personal care and companionship and hospice care.

Adult Day Services (800-642-5119)
Nonresidential support for adults with physical and/or cognitive impairments includes professional health, social and therapeutic services, nutritious meals and valuable social interaction. Offers respite, support and education for family members and caregivers.

Aging & Disabilities Resource Connections — No Wrong Door (802-241-0294)
Partners provide information, referrals and options counseling about long-term care services and supports, respite care and family caregiver support.


Area Agencies on Aging— Senior Helpline (800-642-5119)
The five statewide Area Agencies on Aging respond to the needs of older adults and their families by offering information and services including caregiver support, health insurance counseling, veterans support, transportation, senior nutrition programs including Meals on Wheels, adult day programs, home health services, housing alternatives, and exercise and wellness programs.

Services include adult care and Medicare-certified visits, adult and private-duty nursing, habilitation, personal care and companionship and hospice care.

Elder Services Inc. (802-388-3983)
Offers adult day services along with information about resources for family caregivers.

Older Adults and Persons With Disabilities Transportation Program (802-917-2458)
Provides door-to-door transportation for adults age 60-plus and people with disabilities to medical appointments, adult day facilities, senior meal sites and essential shopping.

Provides cost-effective transportation for older adults, people with disabilities and Medicaid recipients who need transportation to medical appointments and social service providers.

End of Life

Offers planning tools and advance planning consultation services related to end-of-life options, including access to medical aid in dying.

University of Vermont Health Network Home Health and Hospice (802-658-1900; 800-427-1908)
Hospice professionals and trained volunteers manage the medical, emotional and spiritual needs of patients and family members.
