Posted on 11/25/24 by Scott Wegenast

Your Healthcare Journey: Designed for 40203 Neighborhoods

Now Enrolling Participants
The Louisville Community of Care Project is now enrolling 100 participants who live in 40203 neighborhoods to walk on a health care journey. The goal is to reduce disparities in care for
high blood pressure, diabetes, and other chronic conditions.

The Louisville Community of Care Project is an initiative created by pastors in 40203 neighborhoods, medical providers and advocacy groups to address chronic illness in the African American community. The primary objective is to increase access to high quality, person-centered care for African Americans by supporting faith-based organizations to connect members to care resources.

The Coalition to Transform Advanced Care (C-TAC) and pastors are coming together to provide you this opportunity at no cost as funding permits.
